Scott & white care plans, dba baylor scott & white care plan (swcp), was created in 2016 as a texas nonprofit hmo. scott and white health plan (swhp) is the sole member of swcp. swcp was formed with a social welfare purpose with the intent to offer medicare advantage and other commercial hmo plans. together with swhp, swcp's service area encompasses 171 counties and swcp provides coverage to over 15,000 members in north, central and west texas. swcp is part of baylor scott & white health (bswh), a texas nonprofit, faith-based integrated health care delivery system that provides patient care, medical education, medical research and other community services to the residents of north and central texas. as the largest not-for-profit health care system in texas bswh has over 1,300 access points including 52 hospitals, 632 specialty care clinics, 260 outpatient clinics, 164 primary care clinics, 33 pharmacies, 31 ambulatory surgery centers, and over 7,200 active physicians.
