Community care health plan's mission is to develop and demonstrate innovative, flexible community-based approaches to care for at-risk adults in order to optimize their quality of life and optimize the allocation of community resources.we are a non-profit organization with experience in helping adults with long-term care needs live as independently as possible. our medicare/medicaid long-term care programs serve the needs of older adults and adults with disabilities. we coordinate and deliver a full range of supportive services that help them live safely, confidently and with dignity.
What they do
Community care's family care partnership program for frail elders and adults with physical, intellectual or developmental disabilities provides comprehensive medical and long-term care.partnership members have a team of professionals that work to coordinate all aspects of care. the team will assess member needs and develop a cost-effective care plan. partnership members have access to medical care and long-term care services without the worry of insurance bills, co-pays, deductible or insurance gaps. as of the end of 2023 partnership serves 769 members in nine wisconsin counties.
