The long term home health care program (lthhcp) provides a coordinated plan of nursing, rehabilitation and medical care at home to individuals who are medically eligible for nursing home placement. the program also offers case management, social services, and para professional services, among others. the program ceased operations during 2013. see part ix for statement of functional expenses.
The adult day health care program (adhcp) provides medically supervised services for persons with physical and/or mental impairment who are eligible for placement in a nursing home. services include nursing, social activities, physical, occupational and speech therapies, social services and coordination of referrals for outpatient health care. transportation is available to take participants to and from their homes to the program. the program ceased operations during 2013. see part ix for statement of functional expenses.
